In order to make coming to the doctor's office as pleasurable and convenient as possible, we have established the following policies:
· Be on time for appointments! If you cannot make your appointment time, please call and let us know so that we will expect you late or schedule another appointment that is more suitable.
· Unless cancelled 24 hours in advance, with front office personnel, our policy is to charge for any missed appointments at the rate of $30.00.
· If you do not have a current copy of your insurance card, you will be expected to pay for the office visit in full. NO Exceptions!
· Please pay your bills in a timely fashion. Don't expect insurance to pay everything. We accept cash, check, and all major credit cards towards the payment of your account.
· If you or your children's medication needs to be refilled, please call during our normal office hours. Do not wait until the last dose of medication before requesting a refill.
· If your child is admitted into the hospital, we ask that you help us by following our instructions and cooperating with our staff. Compliance in the hospital results in a shorter stay.
· A parent or guardian must accompany children under the age of 10 to the restroom.
· All co-pays and deductibles are due at the time of the visit.
· After medical claims have been filed three (3) times, unpaid balances will become patient responsibility.
· All unpaid balances over ninety (90) days, are due at the time of service, unless prior arrangements have been made with office personnel.
